Getting the skills to live as independently as possible and desired begins in the school years. Taking care of your health is an important part of this. Below are some organizations to help you achieve your independent living goals as an adult.

Central Point of Coordination (CPC) – Call your local county.
The CPC helps direct the managed care (and funding) aspects of independent living and other services forindividuals with mental retardation, developmental disabilities, and sometimes mental illness, as mandated by state law. You may get case management or other services to help you with independent living through your CPC.

DHS can assist with independent living skills services, as well las the funding and-or referrals for such services. Information is also available on state programs to build basic living skills, obtain medical assistance or in-home care/assistance. The Louisa and Cedar Counties offices are open by appointment only. Please call the Muscatine office before you come to these offices.

General Assistance, Trust, and Central Point of Coordination (CPC) services for eligible consumers. Assistance with food, shelter, and medical expenses. Money Management including Social Security payeeships, conservatorships, and guardianships. Determination of eligibility for county-funded services for persons with disabilities. Social Work, Mental Health, and Vocational Services for persons with disabilities. Programs provided include Title 19 Case Management, county Social Work, Community Support Program, Snackers Vocational Assessment and Training, and WINGS Community Employment Services.
